AI loss-of-control incidents top 300 in July

The Centre for Long-Term Resilience’s Loss of Control Observatory logged more than 300 AI incidents in July, almost twice June’s total, in a report based on public user reports. The Observatory’s August interim report says higher-severity incidents rose 7.4× from its first 3.5 months of monitoring to its latest period, while The Guardian’s account notes most cases caused no significant harm. For teams deploying long-running agents, the number is a monitoring signal—not a failure rate: preserve logs, make external actions reviewable, and treat unreported incidents as a blind spot, as the archive’s AISI evaluation incident analysis argues.
